Forms for Students
All forms below are submitted online unless otherwise noted. Most require your UNC Asheville login credentials. If you're unsure which form to use, contact the Office of the Registrar.
- Application for Senior Auditors – For NC residents age 65+ wishing to audit a course on a tuition-free basis
- Declaration form – To add or drop a Major/Minor/Teacher Licensure
- Interdisciplinary Certificate
- Single Course Withdrawal – To withdraw from one or more courses without withdrawing from all classes
- Complete Withdrawal – To withdraw from all courses for the semester
- Hardship Withdrawal – For a student who encounters an unforeseen extraordinary situation such as a serious illness or injury or some other significant personal situation, which prevents the student from being able to successfully continue in their classes. Please view the Hardship Withdrawal Policy for more information.
- Enrollment Notification – For students who need to exit before classes begin or during the first week — no W grade will appear
- Registration Hours Overload – To enroll in more than the standard credit hour maximum for the semester
- Repeat Course Form – To repeat a course and/or request a grade replacement — submit to the Academic Success Center
- Late Add Request – To add a course after drop/add closes through the last day of classes
- Consortium Registration – To take a course at Mars Hill or Warren Wilson while enrolled full-time at UNCA
- Senior Auditor Registration Form – For active students already enrolled the prior semester — use the Senior Auditor Application if you are new or inactive
- Application for Graduation – Apply online through OnePort — this link provides instructions
- Commencement Participation Form – For students who wish to participate in a ceremony other than the one for their graduating term. Must be within 8 hours of completing degree requirements.
- Preferred First Name – Updated in OnePort → Student Records → Update Personal Information
- Release of Non-Directory Information (Online FERPA Release via OnePort) – Instructions for setting up proxy access — no paper form needed
- Request for Replacement Diploma – Must be submitted by the graduate
