Patient
Rights & Responsibilities
The
right to be treated with respect, dignity and consideration.
The
right to information regarding the scope and availability of services.
The
right to know who is counseling, caring for and treating you. The
practitioners name and professional qualifications should
be visible or stated on introduction.
The
right to accurate information, to the extent known, concerning diagnosis,
treatment and prognosis of an illness or health-related condition
in terms you can understand. When it is not medically advisable
to give such information to the student, the information should
be made available to an appropriate person on their behalf.
The
right to receive from the health care provider information necessary
to give informed consent prior to the start of any procedure or
treatment. Except in emergencies, this information shall include
the purpose and description of the procedure or treatment, medically
significant risks and alternate courses of treatment.
The
right to a second opinion regarding diagnosis or treatment and to
change providers if other qualified health care providers are avaialable.
The
right to participate in decisions which are made regarding your
health care treatment. We respect your right to make decisions based
on your personal beliefs and values as well as on available medical
information. This includes the right to refuse treatment to the
extent permitted by law and to be informed of the medical consequences
of this action.
The
right to be informed of any research aspect of your care and to
refuse to participate. Such refusal will not jeopardize your access
to medical care and treatment.
The
right to expect that all communications and records pertaining to
your care will be treated as confidential and require your written
request for releasse except as required by law.
The
right to information about continuing health care requirements,
such as return visits and required medications.
The
right to information about "Advance directives", such
as living wills or health care power of attorney, that express beforehand
your choices for treatment in the event you become unable to make
such decisions.
The
right to application of all your patients rights by the person who
may have legal responsibility to make decisions regarding medical
care on your behlaf. Information on treatment of minor patients
is available from the Student Health Services office manager.
The
right to register a complaint regarding the quality of care provided,
quality of service delivered or any other valid concern.
The
right of information regarding fees for services, particularly,
notification as to what services may involve additional charges.
You have a right to examine and receive a full explanation of your
bill.
As
an active participant in your care, you also have the following
responsibilities,
It
is your responsibility to provide full information about your illness
or problem to allow proper evaluation and treatment.
It
is your responsibility to show courtesy and respect to health care
personnel and other patients.
It
is your responsibility to be informed. Ask your health care provider
sufficient question to ensure appropriate comprehension of your
illness or problem. If you find the care or course of treatment
unacceptable for any reason, please discuss this with us, a member
of the staff, or the medical director.
It
is your responsibility to use identified methods of expressing grievances
and suggestions, whenever indicated, rather than remaining silent
in order to assist the Student Health Services in improving the
quality of their services.
It
is your respsonsibility to not lend your personal identification
to others and to not give medication prescribed for you to someone
else. These actions may lead to serious problems for you and the
other person involved.
You
are responsible for your continued care after you leave the Student
Health Services. You should know when and where to get further treatment
and what you need to do at home to help with your treatment. If
you dont know, ask questions.
You
are responsible for keeping follow-up appointments. If you are unable
to keep an appointment, please cancel or reschedule as far in advance
as possible so that the time may be given to someone else.
You
are responsible for communicating with your health care provider
if your condition worsens or does not follow the expected course.
