Withdrawal Period
For students withdrawing from courses after the
first week, but prior to the end of the ninth week of classes. Note: You must complete both
the top and bottom parts of the form. Multi-part forms are available in the
OneStop Student Services Center located in University Hall 253.

For students withdrawing from
courses after the withdrawal deadline.
Exiting the University
For students seeking to withdraw from all
courses at UNC Asheville during the withdrawal period.
Petition for Exception, Substitution, or Exemption
For students seeking a substitution, exemption,
or exception from General Education, Major, or University Requirements.
Verification of Compliance with Academic Warning Requirements
For students on Academic Warning. Advisor must sign this form verifying
that the conditions have been met.

Request
To request an official transcript be sent to you.
Residency
Application
Application for In-State status for tuition purposes.
Verification of Enrollment
Students complete the top of the
form prior to submitting it to the Office of the Registrar located in the
OneStop Student Services Center, University Hall 253. Students may also
request verification of enrollment online through the National Student
Clearinghouse. For more information students should log in to their
OnePort account or visit
www.unca.edu/registrar.
Visitor
Permission Form
For students who wish to enroll as a visitor at
another institution. Courses must be approved by the Registrar's Office
prior to enrollment at another institution.
Academic Suspension
and Dismissal Appeal
For students who wish to appeal their
suspension or dismissal status.
