Voluntary Group Term Life Insurance

The NCFlex Voluntary Group Term Life Insurance is provided by ING Employee Benefits and underwritten by ReliaStar Life Insurance Company. This plan pays a benefit to your beneficiary(ies) if you die while covered under the policy. This is strictly a life insurance policy that provides a benefit if you die. There is not an accumulated cash value.

Important:

If you enroll in this plan the first time it is offered to you (as a new employee), you may elect coverage up to $100,00 without providing Evidence of Insurability (EOI). An EOI is a way of providing proof of good health. This evaluation may include your current health status, medical and  family history. If you decide to enroll for life insurance coverage or increase your coverage at a later date, you will have to provide an EOI form to ReliaStar Life for approval before coverage can become effective.

Monthly Cost

The monthly premium you pay is based on your age as of your effective date of coverage and the coverage amount you elect.
