Cancer Insurance

 

Cancer Insurance is provided through Allstate Workplace Division. You have two plan options and two levels of coverage to choose from, depending on how much coverage you need. In addition to cancer, this insurance pays benefits for 29 other specified diseases such as Muscular Dystrophy, Multiple Sclerosis, Tuberculosis, and Sickle Cell Anemia. A complete list of the covered diseases is located in the certificate of coverage.

 

Important:

If you enroll in this plan the first time it is offered to you (as a new employee), you may elect coverage on a guaranteed basis without providing Evidence of Insurability (EOI). However if you have had a diagnosis of cancer within the past twelve months, the plan will not pay claims for the first twelve months during which it is in effect.

 

Coverage - Low Option and High Option, and Monthly Cost

Though the coverage offers the same type of benefits and/or services, the amount of coverage differs in the High Option, in most cases. The benefits under the High Option are richer as compared to the Low Option. Please refer to Schedule of Benefits for more details:

 

The monthly premium you pay for cancer coverage is based on the plan you choose and whether you choose to cover yourself or yourself and your family:
